INSTALLER REFERRAL AGREEMENT


Please see if the following statements apply to you:

I realize Joe will provide me with the name of an Individual who works in my area and who has the knowledge, skill and equipment necessary to perform a level of workmanship that meets CRI-105 Installation Standards.

I realize Joe has not verified the status of the Individual's license, insurance, or bond.

I realize Joe has not verified the status of the Individual's credit or criminal record and that it's up to me to do so if I feel it's necessary.

I realize I'm not obligating myself to hire this Individual.

I realize that if I hire this Individual, I'll pay Joe a referral fee of $35 (VISA/MASTERCARD).

I realize that if I hire this Individual, I'll pay the Individual directly for his services rendered.

I agree to hold Joe & Wanders Inc. harmless against any claim relating to the outcome of hiring the Individual.

I promise to provide Joe a completed Report Card (which Joe will provide me) on which I've graded the Individual's performance. I understand that Joe will retain this information in confidence.
